There are 3 basic steps to the application process:

1) Download the Radius Application by clicking the box to your right. Fill it out and return the application as instructed in the document.

2) Upon approval of your application you will be sent a Health Form. Fill out the form as instructed and pay the $25 application fee through SimplyGive on this website.

3) Upon receipt and approval of all required documents you will be sent a Statement of Agreement which will be your final "contract" with Radius International. After receiving your signed Statement of Agreement, Radius will send your letter of acceptance.
