There are 3 basic steps to the application process:

1) Download the Preliminary Application by clicking the box to your right. Fill out and return the application as instructed in the document.

2) Upon approval of the preliminary application you will be sent a Final Application (which will ask for the contact information of 3 References) and a Health Form. Fill out the forms as instructed and return with a $25 application fee.

3) Upon receipt and approval of all required documents you will be sent a Statement of Agreement which will be your final "contract" with Radius International. After receiving your signed Statement of Agreement, Radius will send your letter of acceptance.
